I hereby authorize Current Concepts Institute (CCI) and its assignees to digitally capture the audiovisual components of my lecture(s) as they wish while I am engaged in lectures, seminars, symposia or any other activities. I also grant CCI the right to use these audiovisual components, my name, voice, likeness, and biographical information as well as the right to publish, edit, promote, distribute, copy, and advertise all such items in any educational, editorial, promotional, broadcast, or other program or material whatsoever.

I represent and warrant that I own the copyright to all materials I will be using in connection with my lecture(s). With respect to the content contained in my lecture(s) for which a third party owns copyright (including but not limited to internet images, medical illustrations, and logos), I will endeavor to obtain copyright permission from the owner or remove such content if it is not essential to the subject matter being presented. I acknowledge that I am responsible for the accuracy of the content of my lecture(s) and the content of my lecture(s) is and will remain my intellectual property.

In the event of an error in publication, the sole responsibility of CCI will be to make reasonable attempts to correct such errors in a succeeding edition of a publication within its control. Such correction is in lieu of any other remedy. CCI expressly disclaims all other liability, loss, incidental damage, or consequential damage arising from negligence or any other errors in publishing or other use.

In consideration, I hereby release and hold harmless CCI and any of its principals, agents, servants and employees, officers, directors, licensees, designees, distributors, successors, and assignees from any and all liability arising out of, or in connection with, the taking, recording in any media, publication, distribution, editing or any other use whatsoever of the audiovisual components of my lecture(s). I acknowledge and agree that I am entitled to no compensation for the use of such materials even though CCI (and others) may receive economic benefit, including profits, therefrom. This consent is granted for worldwide use and sale of any resulting multimedia publications, broadcast, or recording and with no time limit.
